LymeAid 4 Kids (LA4K) Application Follow-up Form

[vc_row][vc_column][vc_column_text css=""]Please read instructions below then fill out online form once you have given your LA4K Grant Package to your provider to email us. 1. Grant Package consists of the “Applicant Certification Form” and the “Health Care Provider Form.” 2. Applicant Fills out “Applicant Certification Form.” 3. Applicant gives provider the entire Grant Package, which includes both forms and 1st pg of IRS 1040. 4. The provider is responsible for completing the “Health Care Provider Form” and emailing the entire Grant Package to COTBDAA. 5. [vc_row][vc_column][vc_column_text css=""] The Colorado Tick-Borne Disease Awareness Association (COTBDAA) has been an affiliate member of the Lyme Disease Association (LDA) since it founded as a non-profit in 2016. In honor of the late LDA president Patricia V. Smith, ownership of the LymeAid 4 Kids (LA4K) fund granting program has been transferred to COTBDAA, ensuring continued financial assistance for children nationwide who need diagnosis or treatment for Lyme and other tick-borne diseases. Originally developed with the support of renowned author Amy Tan, the LA4K fund helps children whose families face economic hardship and lack insurance coverage or reimbursement for necessary medical care.
